Job Description
As a Case Care Coordinator, you will serve as the primary point of contact for each individual member, fostering a proactive and holistic approach to their healthcare needs. You will engage directly with members to facilitate intake, develop care plans, coordinate treatment needs, and collaborate with their primary care providers and other treatment facilities. This role requires effective communication with a diverse group of stakeholders across various disciplines, both internally and externally. Under the supervision of the RN Case Manager, you will also prepare inquiries for submission to the Medical Director.
Responsibilities:
Conduct intake calls to warmly welcome members into the program.
Initiate and maintain comprehensive care plans, documenting relevant information and interventions.
Promote adherence to care plans developed in collaboration with members, primary care providers, and other treating professionals.
Assess and address members' unmet needs by coordinating necessary services and treatments.
Identify early risk factors through ongoing assessments and maintain accurate documentation.
Advocate for members and reduce unnecessary barriers to care.
Educate members on program benefits and preventive care strategies.
Stay updated on contract details through internal and external training programs.
Enhance continuity of care by managing relationships with involved providers.
Assist in scheduling future appointments and supporting provider selection for screenings, evaluations, and treatments.
Uphold member confidentiality and adhere to HIPAA guidelines and regulations at all times.
Requirements:
High School Diploma or GED equivalent.
Familiarity with medical terminology or completion of a medical terminology course.
Intermediate proficiency in Microsoft Office (Outlook, Excel, Word).
Excellent written and verbal communication skills, with strong telephone etiquette and interpersonal abilities.
Organizational skills with the ability to multitask and manage time effectively.
Problem-solving skills to ensure timely completion of responsibilities across diverse cases.
Minimum age requirement of 18 years.
Preferred Qualifications:
Previous experience in healthcare-related roles.
Knowledge of CPT and ICD coding systems.
Experience in a call center environment.
National Career Readiness Certificate (NCRC).
Telecommuting Requirements:
Secure handling of company-sensitive documents (if applicable).
Dedicated work area ensuring information privacy, separate from living areas.
Residency in an area with access to UnitedHealth Group approved high-speed internet.
