We’re seeking an MDS Coordinator to join our growing team! Reliability, trustworthiness and consistency with regard to attendance is extremely important to this job. Benefits: * Health/Dental/Vision Insurance * Ancillary policies including AD&D, STD, Cardiac, Cancer, Critical Illness, Hospital Confinement * Whole and Term Life Insurance * Tuition Reimbursement * Paid Time Off * Immediate 401(k) * Unparalleled Corporate Support Responsibilities: * Schedules residents for appropriate admission, annual, quarterly, Medicare assessment or change-of-condition assessments, overseeing communication to nursing and other departments. * Assures compliance with Federal and State regulations regarding completion/coordination of the comprehensive assessment (MDS). * Monitors MDS and care plan documentation on all residents, assuring that complete, orderly chronological and current documentation is in file. * Maintains proficiency in software. * Coordinate and attends regular Medicaid meetings to discuss current residents, pending residents and recipients. * Coordinate and attends all ITTM meetings in order to gather information, communicate changes, maintain and update the clinical records including the Care plan. * Performs care plan updates as needed. * Provide DON, Administrator and pertinent staff with the Quality Indicators Report. * Perform transmissions of the MDS on a timely basis. * Remains current with changes in the industry through attendance at outside workshops and conferences. * Perform Ancillary management, collection and reporting to corporate billing personnel. Requirements: * Licensed Nurse in good standing. * Geriatric nursing background. * Restorative care philosophy, excellent professional skill (both assessment and hands-on and good communication and organization skills). * Working knowledge of nursing home regulations. * Ability to work independently as well as a meaningful member of the team. * Excellent organizational skills in order to coordinate many projects simultaneously.
